About Gastric Sleeve Surgery
Patients who have gastric sleeve surgery feel full faster and longer and lose weight fast. During gastric sleeve surgery, nearly 85% of the stomach is removed from the body, making it an irreversible procedure. The portion of the stomach that remains is made into the shape of a sleeve. This helps to reduce the amount of food that a patient can consume regularly. What’s left of the stomach is connected to the small intestine, so that the nutrients consumed are still absorbed as your food digests. This reduces the risk of malnutrition following bariatric surgery, unlike other surgery types.
The success rate with gastric sleeve surgery is high with long-lasting results. This is dependent however on the patient’s commitment to weight loss and living a healthy and active lifestyle. If you’re interested in this type of treatment, our team of specialists will watch you as your recovery and tend to your needs to ensure your comfort during this time. Once you return home from our bariatric practice, it is important to follow up with a physician about three months following one’s gastric sleeve surgery. A comprehensive blood work analysis is required at this time to ensure the patient is healthy following their procedure.
What Gastric Sleeve Patients Should Expect
Patients will be placed under general anesthesia. Small incisions will be made for a laparoscopy procedure. The reshaping of the stomach begins after these incisions are made. During gastric sleeve surgery, the stomach volume is reduced from approximately 30-40 ounces to approximately 3-4 ounces. The unwanted portion of the stomach is removed and a new stomach is created. Using staples, the new stomach is sealed tightly and all incisions are closed. This successful bariatric surgery type allows you to maintain a healthy weight for a lifetime.
Gastric sleeve surgery is often followed by gastric bypass or duodenal switch surgery if the patient does not reach a specific weight loss goal.
Benefits of Gastric Sleeve Surgery
There are many advantages to having this gastric sleeve surgery at Tijuana Bariatrics Center. The first is rapid weight loss, where most patients reduce their excess body weight by more than half in just four to six months. Patients also feel less hunger, this is because their stomach is smaller after surgery. Patients feel fuller quicker and longer and won’t be hungry in general. The portion of the stomach that produces the hunger hormones, ghrelin, is eliminated during surgery. This surgery is also safer than other treatments because there is less of a risk for infection, serious complications or rejection (LAP-BAND). Other weight loss surgery types require that the small intestine is cut and reattached, however, with gastric sleeve, the intestines stay untouched.
There is also no foreign object put in the body for weight loss to be successful. Surgery takes only one hour, which is less than others such as gastric bypass surgery, making the risk of complications even lower. This procedure also is revisable should the sleeve get stretched over time. This revision is simple and can be performed in a doctor’s office.
Advantages of Gastric Sleeve in Mexico
One big advantage of the gastric sleeve lies in the fact that it does not involve bypassing and of the intestinal tract, thus patients do not suffer the possibility of complications from the intestinal bypass such as intestinal obstruction, anemia, osteoporosis, vitamin deficiency and protein deficiency. This surgery is also suitable for patients who are already suffering from anemia, Crohn's disease and a variety of other conditions that would place them at high risk for surgery involving intestinal bypass.
Another advantage of the vertical sleeve gastrectomy is that it is one of the few surgeries which can be performed laparoscopically in patients who are extremely overweight. This fact makes this surgery attractive to high BMI and high-risk patients.
- Expected Weight Loss Long Term: 50% of Excess Weight Loss after 6 Years (source)
- Gastric Sleeve Surgery has Similar Effect on Type II Diabetes as Gastric Bypass Surgery (source).
Disadvantages of Gastric Sleeve Surgery
One of the disadvantages of the vertical sleeve gastrectomy surgery is that it does not always produce the same reduction in weight as the gastric bypass. In the long term, the vertical sleeve gastrectomy can result in weight regain. This is true of any form of purely restrictive surgery.
Because the procedure requires stapling of the stomach the possibility of leakage and of other complications directly related to stapling, though rare, can occur. In addition, as with any surgery, patients run the risk of additional complications such as post-operative bleeding, small bowel obstruction, pneumonia, infection and blood clots.
The risk of encountering any of these complications is however extremely small and varies from about 0.5 and 1%. Having said this, the risk of death from this form of surgery is about 0 .25% is extremely small. The fact still remains that the overall risk of remaining obese is much higher than doing nothing at all.
Gastric Bypass vs Gastric Sleeve
- Gastric Bypass has better rates of resolving obesity-related comorbidities than Gastric Sleeve. However, Gastric Sleeve has lower complication rates (source).
- Gastric Sleeve has significantly fewer morbidity risks, similar to Lap-Band - while achieving expected weight loss similar to Gastric Bypass.
- Gastric Bypass in Mexico generally costs more than Gastric Sleeve in Mexico.
Gastric Sleeve vs. Duodenal Switch
- Duodenal Switch is much more invasive procedure requiring post-op supplements and vitamins.
- Gastric Sleeve in Mexico is typically less expensive than Duodenal Switch in Mexico.
- Duodenal Switch will typically lead to higher than expected weight loss.
Gastric Sleeve vs. Gastric Banding (LAGB, Laparoscopic Adjustable Gastric Banding)
- Bariatric surgery is an effective treatment strategy for morbidly obese adolescents who have failed medical management. VSG results in greater short-term weight and BMI loss when compared to LAGB. Longer follows up with more patients will be required to confirm the long-term safety and efficacy of VSG in adolescent patients. (Journal of Pediatric Surgery)
- Gastric Sleeve is all-purpose bariatric surgery that shows significant advantages than gastric banding (source).
Am I eligible for Gastric Sleeve Surgery?
As a general rule, the vertical sleeve gastrectomy is best suited to individuals who are either extremely overweight or whose medical condition would rule out other forms of surgery. The vertical sleeve gastrectomy is also commonly used for high risk and/or high BMI patients as a first stage procedure. The patient will often undergo the gastric sleeve procedure, lose weight thus improving their overall condition, and then have the intestinal bypass portion of the surgery performed at a later date. Who Needs Gastric Sleeve Surgery?
Naturally, those who need gastric sleeve surgery are those who cannot lose weight or rid themselves of obesity through dieting and fitness alone. If you are considering this type of bariatric surgery, it is always a good idea to consider other options first. Patients need to understand what gastric sleeve does to one's body.
Gastric Sleeve Complications
It is important for patients to understand the possible complications that can end up arising. A lot of the time, bariatric surgery can give rise to the likes of leakage and even infection. Infection is often a common concern for those who have to undergo an incision. If you are concerned about this then make sure that you keep it well treated and that no bacteria is able to grow in the wound. Other complications that can arise from Gastric Sleeve surgery is internal bleeding. This may end up causing you to have to have a blood transfusion. Bowel obstruction is another complication that you will need to be aware of. Anorexia nervosa and bulimia are also known complications, although with careful handling and care a lot of these complications can be avoided.
Since this bariatric surgery removes most of the stomach, patients will end up experiencing a considerable drop in their weight. This, of course, means that you will not be able to eat as much as you used to. Since this is a permanent procedure, it is something that you will need to get used to. Vomiting and diarrhea tend to be some of the mild side effects that can occur especially when you are getting used to this new way of eating. Esophagus issues are something else that can arise if you are not careful in terms of your eating and how fast you eat. If you feel that there are further complications on the way, it is a good idea to speak to your doctor as soon as possible.
Heavy lifting is not recommended. A long-term maintenance diet is also required, as this will help to keep weight down as well. Vitamin deficiency is a possibility, so supplements will be needed.
It’s important to follow all instructions from one’s doctor and nutritionist. You will be on a liquid diet for a specific period of time. From there, you will incorporate different foods and solids into your diet. It’s important to eat slowly and the required amount of foods set forth in your post-op plan.
The vertical sleeve gastrectomy, like the adjustable gastric band, is classified as a restrictive weight loss surgery. The surgeon removes a major part of the stomach leaving a long tube-like shape or “sleeve”. The new stomach can hold about 60 to 150 cc’s of food or 2 to 3 oz. The Gastric Sleeve surgery, like the Duodenal Switch surgery, leaves the pylorus or outlet valve of the stomach intact. While the shape of the stomach is drastically changed, its function is preserved. Patients need to understand that unlike some of the other forms of surgery, the vertical sleeve gastrectomy is not reversible.
The new stomach continues to function normally resulting in fewer restrictions on foods which patients can consume after surgery, albeit that the quantity of food eaten will be considerably reduced. Many patients see this as being one of the great advantages of the sleeve gastrectomy. Additionally, the removal of the majority of the stomach also results in the elimination of much of the hormones produced within the stomach which stimulate hunger.
